The Patient Portal Login Page is where you will enter a username and password to access your medical information. Once you enter your user name and password, you will press the “Login” button

The login page is used by both patients and advocates/proxies (patient assigned users who have access to the patient’s information) to access the medical information.

If a patient forgets their login or password, the Portal Login Page also has hyperlinks:

• Having trouble logging in – This will open up a pages with contact information for the practice
• Forgot your username –  This will open a page with your challenge questions allowing you to recover your user name
• Forgot your password – This will open up a page with your challenge questions 
• If you do NOT have a Patient Portal account you will need to contact the office at 239-938-2002 to create one
